Lately it has been a challange, dunno if I can keep doing.

Its taking all I can do to keep her hydrated.

Today, has been "I don't want anything to eat".

I finally get her to say she wanted a sandwich. I fixed it, now she's saying she is trying to eat it. Ok, 30 minutes later, she's taken 2 very small bites.

Seems she goes up and down like this each day.

I'm on the verge of just taking her to ER. My call to the home healh nuruse, and that didn't help.

B did you make the appt with her PCP day before?
Not being hungry is not a good reason to take her to the ER when she is eating something unless you are seeing something off with her.
Maybe you can call her PCP's office and see if they can get her some Megace or something along those lines.
Again if she is not ambulating all that much her need for food will diminish, it may look like nothing to you but its not to her do you understand what I mean?
You are always in motion and needing "fuel" if shes not the need is less.
Have you asked her what she wants instead of just pushing something on her? Tell her to give you a suggestion of something she would really like to eat.
