I work with several others to provide 24 hour care for an elderly couple. My question is, should we be keeping a journal type record of what goes on when we do a shift? Eg: he was weak today, did not eat well or all was good today, no problems.
The only time we communicate is when changeing shifts. |

There is NO care plan??Yes you should keep notes for all shifts,otherwise they won't know what's going on,as someone WILL forget to report something.Besides cover your back,if something happens you are covered............

Yes you should keep records but it should say a bit more than he did good today. It should include a real brief summary of events, any changes, no weather reports unless it is relevant to the patient and event. You want to be able to pick up paterns. Just saying he did good today gives you no idea how you got the good. You want to pick up on triggers for good and bad.

You could keep the records as the aides do.They are brief but they give a good detailed report
